MEDICATION ADMINISTRATION

JOB DESCRIPTION & FUNCTIONS

TOPIC: JOB DESCRIPTION FOR MEDICATION ADMINISTRATION

STAFF: RN, LPN OR CMA

Policy: Medications will be administered by the above personnel as ordered by the physicians. Staff member administrating medication is responsible for knowing the action of the medications, average dose and unpleasant effects and contraindications.

Procedure:

1. Comply with the Facilitys personnel and safety policies and procedures.

2. Each medication is to be checked with the MAR to ensure appropriate medication, dose and resident. Medication will be considered on time if it is given within one hour before or one hour after scheduled time. Medication ordered AC will be considered late if it is given PC even if it is within hourly time of administration.

3. PO medications may only be crushed if there is a physician order to do so and it has been verified either with pharmacist or MD or medication book that this medication may be crushed.

4. Adequate fluid should be given with each medication. Residents at aspiration/choking risks or who cannot adequately manipulate the glass to keep from spilling should be closely supervised with any fluids.

5. CMA will assist CNAs with resident cares between medication passes

6. Nurse or CMA is to sign off the MAR.

7. Injections and medications instilled into a feeding tube are to be performed only by a LPN or RN. IV medications are to be performed only by an IV certified LPN or RN.

8. All PRN medications given will be charted as to reason, times, and response on the back of MAR. If necessary, the next shift will follow up with PRN response.

9. Breathing treatments (TX) as scheduled and as needed on PRN basis.

Breathing TXs should be given with the head of bed raised or in a chair.

Staff should stay in the same room as the resident for the duration of each breathing TX, unless he following conditions are met:

a. The resident can reposition themselves safely and taking off the TX by themselves to

avoid any risk of aspiration or suffocation.

b. The resident can effectively use the call light to call for assistance.

c. It is not a new medication being administered.

d. The resident will leave the treatment on until it is finished.

e. The resident has self-monitor order for the breathing treatment.

Breathing treatments are set up and started by a nurse or CMA but can be monitored by a CNA or a float also. Staff monitor residents to make sure that the breathing treatment remains on and functioning correctly for the duration of the treatment.

After treatment, the mask and chamber for breathing TX should be disassembled, rinsed, wiped out, placed in open bag and hung up so it can air dry completely.

10. The medication cart should always be in sight of the nurse or CMA or locked. If the nurse or CMA must leave no medications are to be left on top of the cart and it is to be locked. Likewise, the medication room must always be locked or under direct observation of a nurse or CMA.

11. Any food substance (such as applesauce) that is used for mixing with crushed medications is to be contained. No open containers of food are to be left on medication cart. Any open supplements be covered. (at least with plastic cup or towel)

12. THERE SHOULD NOT BE STAFF FOOD OR BEVERAGES ON THE MEDICATION CARTS.

13. Day shift will be responsible for ordering medications from the pharmacy. Each shift is responsible for maintaining inventory for their own shift and informing day shift of what needs to be ordered.

14. The medication cards are for all schedule medications, eye drops, breathing treatments and oximeter checks.

15. A narcotic count should be done by the charge nurse each shift. Narcotics will be kept in the lock box on the medication carts. Access to the medication carts and medication rooms should be tightly controlled. Narcotics are to be given by nurses only.
