Summary/Objective:\ Reporting to the Targeted Case Management Manager, the Case Manager will complete assessments such as individual and family interviews, arranges for professional evaluations, compiles and/or completes referral or placement packets. Provides service planning and coordination of service functions. Supports community inclusion and advocacy in service delivery. Communicates with all support and /or service providers. Develops and implements the Person Centered Support Plan (PCSP). Maintains records on clients in quarter-hour increments of time spent.
