The SOAR (SSI/SSDI Outreach, Access, and Recovery) Advocatehelpsindividuals at risk for/experiencing homelessness who have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder apply for Social Security disability benefits.
Essential Functions

- Demonstrates skills in the role of case management.
- Completes required training within appropriate time frames.
- Exhibits good listening skills, verbal skills, and written communication skills.
- Must be able and willing to be flexible, a self-starter, punctual, and have the ability to maintain a schedule.
- Must be able to relate to patients empathetically and non-judgmentally.
- Must be proficient at basic keyboarding/word-processing skills.
- Assures clinical and administrative documentation policies are followed.
- Assists in the completion of necessary state reporting on the status of patients.
- Provides consultation and review of potential SOAR cases.
- Conducts meetings with staff who serve as SOAR advocates, for the purpose of effective communication and program coordination.
- Represents agency through attendance and participation in various groups and organizations on the local and statewide level.
- Serves as extended member of the treatment team, backing up and assisting other community case managers as required.
- Maintains responsibility for organizing a schedule and establishing priorities according to existing patient and agency needs.
Professional Development
- Attends workshops, continuing education, and conferences in order to maintain and build skills and expand professional knowledge.
- Actively reads and explores innovative ways to provide services.
- Uses creative knowledge to expand and develop new program ideas, in collaboration with colleagues and supervisor.
- Must complete Case Management Training and provide training certificate to agency within six months of hire
- Become a Certified SOAR Specialist for Kansas.
- Maintain SOAR Specialist certification requirements.
- Participate in continuing education activities and required in-services as requested, to enhance and improve job performance.
Travel
Regular travel within Lyon to provide case management services and may include surrounding counties as needed.
