GENERAL DESCRIPTION

To provide spiritual care in the home to patients of all age groups and perform spiritual assessments, analysis, counseling, and referrals to meet the needs of the patient/family.

Requirements

  1. Develop and adhere to the clients plan for spiritual care, in conjunction with the interdisciplinary team.
  2. Creates continuity of care across all healthcare providers involved in patients care by accurate, timely, and clear documentation. Provides information reflecting patient/family spiritual goals and interventions.
  3. Providing direct spiritual support, consultation, counseling, preparation, and bereavement services for patients, family members, healthcare team, referral sources, and community per agency policy.
  4. Collaborates with staff, clergy and community groups to provide support and education to enhance their sensitivity to the spiritual needs of patients/families and reporting on services as indicated.
  5. Engages in and performs ceremonies and memorial services, such as annual Service of Remembrance and monthly memorials, as needed.

KNOWLEDGE, SKILLS AND ABILITIES

  1. Must be able to effectively communicate, both orally and in writing.
  2. Must have exceptional customer service skills and be able to communicate well with a diverse group of stakeholders.
  3. Must be organized and able to perform multiple, critical tasks simultaneously and frequently.
  4. Must understand issues related to delivery of home care services.
  5. Must have demonstrated ability to be open, sensitive, flexible, and ecumenical.
  6. Must be able to function efficiently and in a positive manner within a high stress environment.
  7. Maintain patient confidentiality in accordance to organizations policy and procedure and HIPPA requirements.
  8. Social and cultural sensitivity appropriate to ethically and economically diverse patient and employee base.
  9. Must be reliable and punctual.
  10. Must demonstrate an appropriate attitude. :::
