This position is responsible for direct service in the community.
- Working one on one proactively with consumers on the treatment plan goals outlined by Case Managers and the Therapist.
- Provide one on one support to consumers in activities of daily living or in maintaining daily routines critical to a stable lifestyle,
- Support consumers in community activities as needed and appropriate
- Promote recovery concepts and the vision of independent living
- Monitor and document progress toward meeting specific treatment plan goals
- Provide support and supervision during consumer crisis
- Support consumer in groups
- Facilitate psychosocial groups as needed
