Case Management Position:
LINK Inc. is expanding and is in need of a Case Manager. This position will be responsible for providing services to individuals with any disability of all ages. Case Managers main goal is to assist individuals with the knowledge and skills that are needed to become and remain independent. Duties include but are not limited to assisting with benefits management, phone calls, paperwork, financial planning for limited income, advocate for/with individuals, learning and understanding safety, housing needs, cleaning and cooking skills, organization, assertiveness, financial management, resources, self-care, communication skills, interpersonal skills.
Position requirements:
Applicants must feel comfortable working independently with individuals and going into those individuals homes to assist them with their needs. Same day travel is required with a company vehicle, however overnight stays will periodically occur with advanced notification. Applicant must have exceptional organizational skills, communication skills, and must be able to multitask.
