Job Title:ACT Individual/Family Therapist

Overview:
The Assertive Community Treatment (ACT)Individual and Family Therapistis responsible forproviding individual and group counseling services to individuals with substance use and co-occurring mental health disorders.This position will work as part ofthe ACTmultidisciplinary treatment team to provide integrated, evidence-based care that addresses both mental health and substance use concerns.
Primary Responsibilities:
- Conductcomprehensive andthorough clinical assessments of individuals referredtothe ACT program.
- Collaborate with the treatment team to develop individualized treatment plans based on assessment findings.
- Utilize evidence-based therapeutic modalities to address the mental health needs of clients.
- Provide individual therapy sessions to clients, focusing on skill-building, symptom management, and recovery goals.
- Facilitate group therapy sessions aimed at enhancing coping skills, social support, and overall well-being.
- Respond to crisis situations promptly, employing de-escalation techniques and crisis intervention strategies.
- Collaborate with the crisis intervention team to develop and implement crisis prevention plans.
- Work closelywithACT team membersto ensure integrated and coordinated care.
- Participate in treatment team meetings to discuss client progress, treatment adjustments, and discharge planning.
- Monitor client progress in treatment, using outcome measures and other tools to assess effectiveness of treatment, and adjust the treatment plans as needed with client input.
- Maintainaccurateandtimelydocumentation of all clinical interactions and treatment plans in compliance with agency policies and relevant regulations.
- Engage with community resources to enhance the support network available to clients.
- Provide education and outreach to promote mental health awareness and reduce stigma in the community.
- Other duties asreasonablyassigned.
Supervisory Responsibilities:
This position reports to the ACT Team Leader. This is a non-supervisory position.
Physical Demands:
- Based out of the Hiawatha Community Services building.
- Requires regular travel in the community setting using a company vehicle.
- Work may involve exposure to potentially volatile situations and requires the ability to respond effectively to crisis situations.
