The Role You Play
As the Waiver Facilitator, you will be responsible for acting as the primary point of contact for Pawnees clients who utilize or qualify for the State of Kansas Home and Community Based Services (HCBS) Severely and Emotionally Disturbed (SED) waiver. In this role, youll be responsible for tracking services and other clerical responsibilities to meet Kansas requirements. Youll also be expected to become a subject matter expert on the program and act as support and consultation for clients and staff alike.
- The Day-to-Day
- Monitor and track SED waiver services to comply with State
requirements
- Track and submit waiver data to the state
- Monitor service outcomes to determine if current services are in treatment plan
- Act as the point of contact for SED waiver referrals, and assess cases to determine the level of services needed
- Facilitate quarterly plan of care reviews with clients and their families taking responsibility for scheduling and confirming appointments
- Collaborate with all levels of care from the child, to their caretaker, to other providers inside and outside of the organization
- Monitor and track SED waiver services to comply with State

- Update and maintain the organizations waiver spreadsheet
- Take point on the program, providing support and guidance to staff, community members, and clients about the waiver program, application process, and eligibility requirements
