We are looking for a Case Manager to play a key role in furthering our mission in our Disability and Advocacy Services area.

Summary/Objective:

Reporting to the Targeted Case Management Manager, the Case Manager will complete assessments such as individual and family interviews, arranges for professional evaluations, compiles and/or completes referral or placement packets. Provides service planning and coordination of service functions. Supports community inclusion and advocacy in service delivery. Communicates with all support and /or service providers. Develops and implements the Person Centered Support Plan (PCSP). Maintains records on clients in quarter-hour increments of time spent.

Education/Experience:

Bachelor's Degree and a minimum of at least six months experience in the field of services to individuals who are developmentally disabled or relevant experience in the field, which may substitute for any portion of a Bachelor's Degree at a rate of six months of experience for each semester of college.

Preferred: Behavior Management background. Experience developing behavioral plans, collecting data etc.

Essential Functions:

  • Assessments, such as: individual and family interviews, completion of one or more assessment instruments, arranging for professional evaluations or assessments, compiling or completing referral or placement packets.

  • Service Planning, such as: contacts, (meetings, telephone calls) concerning an individual’s pay, reviewing and monitoring the plan and the individuals progress, providing or arranging for training of staff specifically related to a particular person and quality assurance of service provided.

  • Service Coordination, such as: providing and/or sharing information about services and resources, referral for services, follow-up on referrals, oversight of services, evaluating the individual’s satisfaction with supports and services, and linking the individual to community resources, supports and services.

  • Community Inclusion, such as; researching available services, contacting and accessing integrated supports and services, developing and accessing natural supports and services, such as volunteers, neighbors, colleagues, family, and time-limited independent training of the individual.

  • Advocacy, such as: securing SSI or SSDI, establishing special plans for work, assisting in representing the individual at meetings and hearings, etc.

  • Responsible for coordination of transportation service.

  • Communication with the individual, designated representatives, support or service providers or other relative to the service plan and documentation of services provided.

  • Develop and implement the Person Centered Support Plan (PCSP).

  • Maintain records on clients in quarter-hour increments of time spent.

  • Alternate for Behavior Management Committee.
