\ \ Program/Project Coordinator\ \ Department: 3670055010 Health & Human Sciences Dean\ Job no: 517698\ Employment type: Staff Part Time (Unclassified - Term)\ Location: Manhattan, Kansas\ Worksite: Hybrid eligible (partially remote, must reside in US)\ Categories: Administrative / Professional, Adult Development\ Pay Grade: 08A\ \ \ About This Role:\ The Center on Aging (COA), within the College of Health and Human Sciences, seeks a Program Coordinator to join their team. This position is responsible for supporting the PEAK project through providing direct education, consultation, evaluation, and feedback to providers in homes enrolled in the PEAK program. This position reports to the COA Interim Director.\ \ Responsibilities include, but are not limited to:

  • Review and provide feedback on participating homes\' action plans
  • Provide consultation on person-centered care (PCC) practices to enrolled homes, as needed
  • Engage in education and/or presentations about PCC and PEAK program
  • Plan and promote the annual PCC Conference in collaboration with the PEAK Advisory Group
  • Assist with the development and revision of program content
  • Participate in PEAK Advisory Group meetings and workgroups
  • Participate in planning and execution of participating homes\' evaluations of PCC practices, via Zoom and in-person
  • Assist with Zoom meetings and content development
  • Travel to participating homes in Kansas and conferences, as needed
  • Assist with monthly lunch and learn sessions, and recruitment and retention activities
  • Work with program contractor, KDADS, as needed
  • Contribute to contract development and service provider contract management
  • Respond to participating homes, as needed, by email, phone, and/or Zoom

\ This is a part-time, benefits-eligible position. The Center\'s academic programs encompass an undergraduate minor, secondary major, Master\'s degree, and graduate certificate. We also serve older Kansans and their families through community engagement and outreach and our aging services research projects. At Kansas State University, we are a transdisciplinary unit housed within the College of Health and Human Sciences, with collaborative relationships across several academic departments and centers both within the College and across the University. Our faculty and staff have existing partnerships with K-State Research and Extension programs, regional universities, the Kansas Department of Aging and Disability Services (KDADS), the Kansas Department of Health and Environment (KDHE), various national long-term care organizations, and national research partnerships.\ \ PEAK (https://www.hhs.k-state.edu/aging/research/peak20/) is a Medicaid pay-for-performance incentive program for nursing homes in Kansas. The Kansas Department of Aging and Disability Services (KDADS) contracts with the Center on Aging to administer this program. The KSU PEAK team enrolled homes across Kansas to successfully implement person-centered care through education, training, techni al assistance, and evaluation. PEAK has produced a fourfold increase in the number of Kansas homes fully implementing person-centered care directly resulting in improved clinical and quality of life outcomes for residents.\ \ The College of Health and Human Sciences is home to six academic departments (Applied Human Sciences; Food, Nutrition, Dietetics & Health; Hospitality Management; Interior Design & Fashion Studies; Personal Financial Planning; and Kinesiology), the Center on Aging, Physician Assistant program, and a Nursing partnership with Wichita State University.
