As a Field-Based Case Manager, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Location and Travel: Travel up to 75% and manage members in Wyandotte & Johnson counties.

If you reside within 45 miles of Wyandotte or Johnson County, Kansas, you’ll enjoy the flexibility to work remotely * as you take on some tough challenges.

Primary Responsibilities:

  • Visit Medicaid members in their homes and/or nursing homes or other settings

  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction

  • Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels

  • Assist members with the transition from a care facility back to their home

  • Case Manage members from a Holistic perspective

Required Qualifications:

  • Must meet one of the following:

  • Current, unrestricted independent licensure as a Social Worker

  • Four year or higher degree in a behavioral health related field (e.g., psychology, counseling, etc.) plus 3+ years of direct experience

  • LPN with 5+ years of direct experience

  • 3+ years experience directly serving individuals with behavioral health conditions (mental health/substance use disorders)

  • 1+ years of experience with local behavioral health providers and community support organizations addressing SDoH (e.g., food banks, non-emergent transportation, utility assistance, housing / rapid re-housing assistance, etc.)

  • 1+ years of experience with MS Office, including Word, Excel, and Outlook

  • Access to reliable transportation and the ability to travel within assigned territory to meet with members and providers

Preferred Qualifications:

  • Experience/additional training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care

  • Bilingual in Spanish or other language specific to market populations

  • Background in Managed Care

  • Experience working in team-based care

  • Experience in serving individuals with co-occurring disorders (both mental health and substance use disorders)

Physical Requirements:

  • Ability to transition from home office to field locations multiple times per day

  • Ability to navigate multiple locations/terrains to visit employees, members and/or providers

  • Ability to transport equipment to and from field locations needed for visits (ex. laptop, stethoscope, etc.)

  • Ability to remain stationary for long periods of time to complete computer or tablet work duties
