THE RUNDOWN:

This position is an integral part of the patient care team. The Long-Term Care (LTC) Coordinator will serve as liaison and primary contact between CHC/SEK and the long-term care facilities and those patients within the LTC facility CHC/SEK provides services to. This position will help facilitate patient admission and/or discharge, service as the point of contact for the LTC facility staff and administration, coordinate visits between the LTC facility and CHC/SEK provider(s), as well as working collaboratively with the LTC staff to ensure patient needs are met. The Long-Term Care Coordinator will focus on ensuring continuity of care for all patients served.

Requirements

::: {bind="html: Job.Requirements"} WHAT YOU\'LL BE DOING:

  • Ensure patient records are current within CHC/SEKs electronic health record and maintain timely communication with provider.
  • Continually maintain a dedicated fax server in a timely manner. This entails assigning incoming documents such as faxed refill requests, patient records, orders requiring signatures, discharge instructions, and patient updates from facilities. The LTC coordinator will complete any requests that require a nurse or provider response.
  • Communicate with and assemble documentation from outside providers.
  • Accompany provider to facilities as needed.
  • Manage incoming calls and messages as related to ongoing patient care.
  • Assist provider with patient admission and/or discharge from hospital and/or LTC facility.
  • Place orders and referrals as directed by provider in electronic health record.
  • Facilitate referrals.
  • Ensure safety and medical needs are met prior to patient discharge to home. This may entail education on fall risks and other safety hazards as well as making sure patient has the necessary supplies, equipment and home services needed to return home safely.
  • Perform all duties and services in full compliance with CHC/SEK policies and procedures.

WHAT YOU\'LL NEED:

  • Possession of a valid Kansas Registered Nurse License or multistate license in the nurses resident state.
  • Basic Life Support certification required;
  • Preferably two years of Long-Term Care experience;

WORKING CONDITIONS:

While performing the duties of this job, the employee is regularly required to situ; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is occasionally required to stand. The employee is occasionally required to walk. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception and ability to adjust focus. Computer work is frequent. Good lighting and comfortable temperature of ventilation at all times. :::
