Care Coordinator (LPN/RN)

The Care Coordinator is responsible for providing care coordination, case
management, and/or wellness visits to SFHC patients and individuals/families
served.
Required qualifications include a current LPN or RN Kansas state license, CPR
certification, current Kansas drivers license and proof of appropriate auto
insurance.
Screening and interviews will be conducted on a rolling basis.
