Job Duties

Provides intermittent skilled nursing care to patients in their place of residence in accordance with agency policy and procedure and State and Federal Regulations. Performs direct and indirect nursing care to patients in their homes under the direct supervision of the Registered Nurse. Assists the Registered Nurse in carrying out the plan of care. Performs ongoing assessments during visits, documents nursing interventions and findings, and communicates significant findings with the supervising RN and the physician. Performs nursing skills to include obtaining vital signs with each visit,medication set-up; administration of medication-oral, topical, rectal; administer vaccinations; perform venipuncture for lab draws; diabetic nail care; treatment and evaluation of wounds;administers injections subcutaneous and intramuscular, changes indwelling foley and suprapubic catheters on a regular schedule and as needed, and performspatient teaching as outlined by the care plan and directed by the Registered Nurse. Reports care performed and observed to the supervising RN, and documents in the patient medical record timely and efficiently. Communicates effectively with the patient, patient representative, family, other community agencies, and physicians.Has knowledge of and compliance with Department policies and procedures. Must complete the National Incident Management System training. Maintains certification in CPR and yearly blood-borne pathogen training. Reports for work during public health emergencies as needed. The job may require weekend and/or evening work and be on-call. Perform other duties as assigned.

Minimum Qualifications

  1. Have graduated from an approved school of practical nursing.

2. Must meet licensing requirements by the Kansas State Board of Nursing for Licensed Practical Nurses and have a current licensure for the State of Kansas.

3. Must have good communication skills to work with staff and the public in a congenial manner.

4. An ability to maintain a professional demeanor.

5. Must have basic knowledge of PC software.

6. A proven ability to plan and organize work.

7. Must be flexible and be able to adapt to different tasks.

8. Must have a valid Drivers License and must provide own transportation

Physical Requirements Determined Work Level: Level 3 Medium Work Exerting 20 to 50 pounds of force occasionally, and/or up to 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly. Ability to communicate by means of the spoken word. Ability to hear. Must be able to lift to 50 pounds. Must have fine and gross motor ability. Must be able to operate computer, telephone, and automobile. Possible exposure to infectious pathogens, blood, body fluids and hazardous waste. May be exposed to a variety of work environments including outside of Health Department buildings in all weather conditions Veteran\'s Preference
