Job Title: Pharmacist
Job Description
We are seeking a dedicated and detail-oriented Pharmacist to join our team. You will evaluate physician medication orders for appropriateness, make recommendations for therapy changes, maintain patient databases, and ensure the ordering and evaluation of laboratory tests for high-risk conditions. You will collaborate with healthcare teams to provide clinical and technical support and document all communications in the medical record. Additionally, you will implement policies for the safe procurement, storage, distribution, and disposal of drugs, inform the Pharmacy Manager of medication errors, and assist staff with drug-related procedures.
Responsibilities
Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
Make recommendations for therapy changes to physicians and facility staff.
Maintain patient/resident databases including medical history, clinical and laboratory data, and concurrent medication.
Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with healthcare team members.
Collaborate with the healthcare team to provide clinical and technical support.
Document all communication with healthcare team members, patients, and their families in the medical record.
Implement policies and procedures for the safe procurement, storage, distribution, and disposal of drugs and biologicals.
Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and policy.
Inform the Pharmacy Manager of medication-related errors.
Assist facility/agency staff with procedures relating to the safe and effective handling of drugs.
Perform other tasks as assigned.
Essential Skills
Bachelor’s Degree in Pharmacy; Pharm D preferred.
One year of experience in long-term care.
Licenses/Certifications: KS License.
Detail-oriented and goal-oriented.
Team player with strong oral communication skills.
Additional Skills & Qualifications
Pharmacy and PharmD certifications.
Experience in a long-term care setting.
Work Environment
The shift is from Noon to 4pm on Saturdays and Sundays only. This is a closed-door pharmacy environment where you can focus on fulfilling the pharmaceutical needs of long-term care and geriatric living clients. It offers a non-retail setting with a focus on production, fulfillment, and warehouse activities. Our organization is in high growth mode, providing opportunities for career progression.
Pay and Benefits
The pay range for this position is $65.00 - $75.00/hr.
