GENERAL DESCRIPTION:

Generally, provides overall case coordination for assigned case load by coordinating all necessary services. Completes required paperwork timely.

DUTIES:

E = essential function, M = marginal function

  1. Provides overall case coordination for up to 8 unaccompanied children (UCs). May be more than 8 youth during influx. Coordinates all necessary services. (E)
  2. Completes all intake services, including an initial intake assessment within 24 hours of admission. (E)
  3. Completes all required program documentation for each UC. (E)
  4. Completes and updates Case Tracker by noon of each Monday. (E)
  5. Meets at least once each week with each youth assigned to his/her caseload, provides accurate and detailed progress notes, facilitates release planning and communication. (E)
  6. Ensures safety of all youth. (E)
  7. Manages a high-turnover caseload created by incoming placements and releases. (E)
  8. Manages reunification process for each child. (E)
  9. Coordinates each case with the clinician, Lead Case Manager, and GDIT Case Coordinator, (and at times CFS and FFS) to review progress, concerns, and make recommendations for release. (E)
  10. Participates in weekly staffing conference call with GDIT Case Coordinator and Contract Field Specialist (CFS) and FFS. (E)
  11. Responds to child emergencies within or outside work hours. (M)
  12. Completes annual training of all required credit hours (CEUs). (E)
  13. Complies with rules and regulations governing access to Protected Health Information under HIPAA. (E)
  14. Responds in a positive and respectful manner to diversity among the youth cared for by the agency and staff. (E)
  15. Ensures each youth in the home has all of the rights provided in The Rights of Children in Care at The Villages Inc. as adopted by the Board, and as enumerated in the Youth Handbook. Ensures that each youth understands Zero Tolerance, Garza Notice, and all documents and rights detailed in the Legal Resource Guide, both at the time of admission and discharge. (E)
  16. Sends the Family Reunification Packet to the sponsor in a timely manner. Ensures complete receipt of requested documents from sponsor in a timely manner. (E)
  17. Maintains the Office of Refugee Resettlement (ORR)/Administration of Children & Families( ACF) data base and ensures all placement, transfer and release requests as well as all significant incident reports are processed through this web-based (UC portal) system. (E)
  18. Makes a recommendation that carries significant weight as to whether a child is released from the program and to whom the child is released. As the primary professional managing the case, shares case management plan with the federal government representative for final authorization/approval. (E)
  19. Collects required information and uses professional, independent judgment to determine whether the intended placement is safe and appropriate. (E)
  20. Completes thorough and appropriate research into the background of the proposed sponsor as well as the conditions of the living environment and uses professional judgement to determine whether the proposed placement is in the best interest of the child. Using professional judgement and guidance from the UC program, evaluates and compares all possible courses of action with regard to the placement of a child with a potential sponsor. (E)
  21. Maintains appropriate, professional, satisfactory and harmonious relationships with the public, fellow employees and youth. (E)
  22. Completes 7 day on-call rotation (every 6-8 weeks) during which time completes after-hours intakes, orientations, and Significant Incident Reports (SIRs).
  23. Ensures quality maintenance of case management records, both the physical file and electronic uploads in compliance with ORR guidelines. (E)
  24. Provides line of sight and sound supervision of youth at all times. (E)
  25. Accepts additional duties as assigned. (M)
