Job Summary
The Program Delivery Lead will partner with the National Director of Program Delivery to design, communicate, and implement strategic plans to impact patient care transitions across the primary care continuum and quality improvement activities to address high risk, high-utilizers/high-cost patient management. The Program Delivery Lead works on problems of diverse scope and complexity ranging from moderate to substantial and has a strong understanding of how such business complexities impact caring for seniors in a value-based care model.
The Program Delivery Lead will partner with various leaders regarding implementation, planning, and execution of programs and advises the Director of Program Delivery and significant functional strategies. The Program Delivery Lead will also exercise independent judgment and decision making on complex issues regarding job duties and related tasks, and works under minimal supervision, using independent judgment on analysis of variable factors and determining the best course of action.
This is a high-profile role requiring a strategic and innovative mindset. The successful candidate will carry out the following duties and responsibilities:
Major Duties and Responsibilities
Monitors progress and performance against strategic care integration project plans and care integration programs roadmap. Includes the ability to be forward thinking while not losing site of immediate priorities.
Owns, designs, and executes specifications related to budgets, forecasts, staffing ratios, data analysis, data modeling, KPIs, technology enablement and integrations. Includes ability to analyze complex business problems using qualitative and quantitative inputs.
Establish collaborative relationships and work with strategic key stakeholders, including clinical teams, technology and data analytics leaders, and care integration team (CIT) leadership to support program and project implementation and facilitate adoption of new strategies and tactics.
Manages special projects at the request of Director of Program Delivery on an ad-hoc basis to support care integration programs strategic goals across range of topics, including operations and processes.

Required Qualifications
Bachelor’s degree in health care administration, business administration, nursing, social work, or a related field.
3-5 years’ experience of management experience preferred in VBC/experience working within case management programs.
Preferred Qualifications
Master’s degree in health care administration, business administration, nursing, social work, or a related field.
Experience working in a complex value-based health care organization.
Knowledge/Skills/Abilities/Competencies Required
Excellent communication and interpersonal skills with the ability to communicate effectively with stakeholders at all levels.
Strong analytical and problem-solving skills with the ability to develop and implement metrics to measure the effectiveness of care transitions.
Understanding of clinical operations and ability to guide priorities, process development and implementation.
Understanding of quality and value drivers in full risk care delivery, ideally in Medicare/seniors
Demonstrated ability to work collaboratively with clinical and operational leaders across a complex health care organization.
Knowledge of health care regulations and policies related to care transitions and post-acute care management.
Proficiency in tracking and evaluating process/system outcomes and performing and monitoring rapid cycle process improvement.
Excellent project and program management skills
Demonstrated ability for complex clinical decision making.
Ability to multi-task in a fast-paced work environment
Excellent organizational skills
Ability to independently manage projects and maintain timelines for completion
Compassion and desire to advocate for patient needs
Computer skills in word processing, database management, spreadsheets, and report writing.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$115,200 - $158,400 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
