As an HSS Clinical Coordinator, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Location and Travel: Travel up to 75% and manage members in Shawnee county and surrounding areas.

If you reside locally to Shawnee County, KS, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:

  • Visit Medicaid members in their homes and/or other settings

  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction

  • Complete all assessments per contract requirements and identifying any unmet needs

  • Provide care coordination to members from a Holistic perspective

Required Qualifications:

  • Associate degree

  • 4+ years of experience working with the community health setting in a health care role

  • 1+ years of experience working with MS Word, Excel and Outlook

  • Experience in long-term care, home health, hospice, public health or assisted living

  • Willing or ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or provider’s offices

Preferred Qualifications:

  • Licensed Social Worker or clinical degree

  • 2+ years of case management experience

  • 1+ years of experience working with Medicaid Waiver services

  • Field Based work experience

  • Experience with arranging community resources

  • Experience with electronic charting

  • Background in managing populations with complex medical and behavioral needs
