Nurse Disease Management I

Preferred location: Norfolk, VA or Remote

The Nurse DiseaseManagement I is responsible for participating in delivery of patient education and disease management interventions and for performing health coaching for members, across multiple lines, for health improvement/management programs for chronic diseases. Primary duties may include, but are not limited to:

  • Conducts behavioral or clinical assessments toidentify individual member knowledge, skills and behavioralneeds.

  • Identifies and/or coordinates specific healthcoaching plan needs to address objectives and goals identified duringassessments.

  • Interfaces with provider and other healthprofessionals to coordinate health coaching plan for themember.

  • Implements and/or coordinates coaching and/orcare plans by educating members regarding clinical needs and facilitatingreferrals to health professionals for behavioral healthneeds.

  • Uses motivational interviewing to facilitatehealth behavior change.

  • Monitors and evaluates effectiveness ofinterventions and/or health coaching plans and modifies asneeded.

  • Directs members to facilities, communityagencies and appropriate provider/network.

  • Refers member to catastrophic casemanagement.

Specifics ofrole

  • Work from home position anywhere within the US,preferred location Norfolk VA

  • (If you are within 50 miles of Norfolk Office,will be required to do on-site training and come in office when needed based onneeds of business)

  • Full time position (Monday - Friday 40 hour workweek)

  • You must have a current RN license in one of thefollowing states to be considered: Florida, Georgia, West Virginia, or state ofresidence is a multi-state license."

  • This position will require you to obtainadditional licenses, you must be willing to do this with assistance from thedepartment.

Hours of Position(no flexibility)

  • Monday - Friday 8:30 AM to 5:30 PMEST

  • Requires an AS/BS in nursing; 2 years ofcondition specific clinical or home health/discharge planning experience; or anycombination of education and experience, which would provide an equivalentbackground.

  • Current unrestricted RN license in applicablestate(s) required.

  • Prior case management experiencepreferred.

  • Proficiency with MS Word, Excel andOutlook.
