Position Summary & Responsibilities


Position Summary:


The position (K0242922) will coordinate and oversee the planning and implementation of activities directly related to Strategy 4 (Mobilize partners and collaborators to advance health equity and address social determinants of health as they related to COVID-19 health disparities among populations at higher risk and that are underserved) of the Health Equity Grant. With multiple internal and external partners, the program manager will oversee the ongoing promotion and the development of Community Health Worker programs in funded clinics. Program Manager will work with partners at the state and local levels to align Community Worker programs and promote locally and statewide.


Job Responsibilities may include but are not limited to the following: Work with selected local clinic programs and partners to assure state level goals and objectives defined in the Health Equity Grant Workplan are being met within the timeline defined by CDC. With internal and external partners, facilitate program evaluation, develop reports and participate in the planning of Quality Improvement projects.


Qualifications


+ Education:


+ Bachelor’s Degree, Health Promotion, Health Education, Nursing, Social Work, Public Health preferred, experience may be substituted when appropriate.


+ Licensing & Certification:


+ Valid Driver's License - Incumbent is required to have and maintain a valid driver's license when operating a state vehicle, a private vehicle, or a rental vehicle for the benefit of the State.


+ Minimum Qualifications:


+ 2 years of experience planning, developing, implementing, coordinating and/or providing social or human services to the public or community. Education may be substituted for experience as determined relevant by the agency.


+ Preferred Qualifications:


+ Word, Excel, PowerPoint, Access


+ Proofreading, editing, attention to detail


+ 2 years’ experience in technical assistance, program evaluation, training, and/or grants management


+ 2 years’ experience coordinating or providing support to state or community based coalitions


+ Experience preparing grant proposals and reports


+ Excellent interpersonal skills, ability to build and maintain professional relationships


+ Experience working in or with health care field and/or experience working with Community Health Work
