Offer individual and group counseling to caregivers caring for older adults to address mental health and caregiver issues. This includes performing clinical assessment, notes for records, and managing data for grant reporting. Provide community psychoeducation and outreach regarding the importance of mental wellness for older adults, to decrease the stigma of therapy and treatment and to help individuals and professionals identify risk factors in older adults. Provide consultation and support for agency staff.Service ProvisionConduct mental health assessments in the office and the community to assess immediate mental health needs including history, presenting problems and diagnoses using DSM-V criteria and development of a treatment planProvide short term and group therapy services to older adults and caregiversProvide consultation to agency staff regarding client issuesProvide outreach and education to older adults, caregivers and professionals in the community to increase program referrals and community knowledge of the service and importance of mental health and wellness.Specialized ActivitiesCoordinate and assist with special projects, new programs which enhances or aids in the delivery of services to older adults and caregiversActivities may include but not be limited to crisis assessment program for seniors, depression screenings, family consultation as appropriate, caregiver or specialized older adult support group facilitator, referrals to other agencies or internal programsCommunity education and outreach to older adults, caregivers and professionals to promote the importance of mental wellness but also identify referral resourcesDocumentationComplete progress notes in the management information system in accordance program/department procedures of goal-directed therapy and client response to treatment within prescribed timelinesComplete Notes for Record, authorization for release of information or records and other documentation as required.Submit reports to program manager for grant reportingObtain an NPI number for billing purposesMinimum Qualifications: Master's Degree with a LSCSW in Social work from an accredited college or university. One (1) year working with older adults. One (1) year experience providing individual or group therapy. Must be licensed by the Behavioral Sciences Regulatory Board as a LSCSW. Must have a valid Kansas Driver's license and current proof of automobile insurance.
