Summary
The Certified Home Health Aide (HHA) is responsible for assisting patients and their family members with personal care tasks as assigned and supervised by the registered nurse.
Responsibilities
- Assists in maintaining the patients physical comfort and hygiene, as directed by the RN.
- Provides appropriate type of bath according to the patients condition and HHA care plan.
- Provides oral hygiene, as directed.
- Provides personal grooming, as directed.
- Promotes personal grooming, as directed.
- Promotes optimal skin integrity by providing skin care, as directed.
- Performs non-skilled nursing procedures, as ordered by the physician and directed by the RN.
- Assists patient with ambulation, transfers, and range of motion, as directed.
- Performs all tasks assigned per the patients plan of care; report to RN any tasks not completed at time of service.
- Maintains a clean, safe and comfortable environment for the patient.
- Performs housekeeping duties, when directed by the RN.
- Changes patients bed linens, as directed by the RN.
- Demonstrates appropriate us of standard precautions.
- Contributes to continuity of care and effective communication between hospice team members.
- Accurately assesses and records patients vital signs when directed by RN.
- Reports significant physical and/or emotional changes in patient/family condition to the appropriate interdisciplinary team member and/or RN.
- Accurately document care provided in a timely manner per company policy.
- Communicates in a clear and courteous manner with patients/families and staff involved with patients care.
- Complies with the Federal HHA Standards by attending a minimum of 12 hours of in-service per calendar year.
