DIRECTOR OF NURSING

Job Purpose: The Director of Nursing is a key administrator in the healthcare system. They collaborate with other healthcare professionals while overseeing the day-to-day operations of the nursing department. The Director of Nursing is also responsible for performing routine and non-routine clinical duties to assist Providers and other healthcare practitioners with patient care to ensure quality and efficiency. This position performs all clinical duties in accordance to state law and under the direction of a Physician.

Patient-Centered Medical Home Essential Functions:

1.Patient Population Management:

a. Takes patient phone calls, answers patient questions and requests, provides information to patients, and returns patient messages.

b. Reviews prior authorization requests for medication to ensure all necessary information is provided; processes prior authorizations and accurately enters into database.

c. Maintains patient confidence and protects operations by monitoring confidential information processing.

2.Care Coordination:

a. Provides direct patient cares as described in state regulations for RN, in all cases involving nursing services, including initial evaluations visits as applicable and re-evaluation of the patients needs on a regular basis, provides nursing services, treatments, diagnostic procedures requiring specialized skills and assumes full responsibility for each assigned patient.

b. Coordinates services for patients, as needed, to other agencies and health care providers as appropriate.

c. Supervises all vaccines and immunizations given by a Medical Assistant or Certified Nursing Assistant.

d. Oversees and reviews patient medication refill requests.

3.Self-Management Support:

a. Manages workforce and staffing levels to guarantee compliance with quality-of-care standards and appropriate ratios.

b. Complies with all applicable federal, state, local and clinic rules, regulations, protocols and procedures.

c. Collaborates with various departments, including medical admissions, operations, and quality management, fostering effective communications and coordination.

d. Participates in HealthCore Clinic task force in preparing for outside audits (HRSA, FTCA, etc.).

e. Work with the Chief Clinical Officer/Medical Director closely to manage medical staffing issues.

4.Communication Skills:

a. Delegates tasks effectively working with and through others as needed.

b. Assists with overseeing the Certified Nurse Aides, Medical Assistants, Phlebotomists, Early Detection Works Case Manager/Community Health Worker and Lab staff by providing guidance and resolving issues, administering performance management, evaluations, discipline, support, and coordination of schedules for staff and approving timecards.

c. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks and participating in professional societies.

d. Attends and participates as a team member on the Risk Management Committee and at lea t one HCC quality team initiative team such as quality improvement, safety, and/or employee recognition teams.

e. Participates in appropriate health promotion/disease prevention activities both on-site and off-site as required.

Additional Essential Functions include the following:

1. Consistently checks temperatures on the vaccine fridges and freezers to ensure they are compliant with required temperature levels, logs levels.

2. Maintain sufficient vaccine stock through timely and cost-effective ordering. Stay up to date on vaccine changes and inform/educate all medical staff. Maintain VFC-mandated records and attend VFC conferences yearly, to maintain certification with program.

3. Maintain medical supplies and medications in an orderly, cost-effective manner, according to policies and procedures. Work with Dental, Pharmacy, and Behavioral Health to centralize supplies and decrease overlap in purchasing.

4. Pulls International Health Assessments (IHA) records from the Electronic Disease Notification (EDN) system.
