JOB DESCRIPTION
POSITION TITLE : Housing Case Manager
REPORTS TO: Director of Homeless Services
POSITION OVERVIEW : The Housing Case Manager provides case management and related services to eligible participants in designated Residential Care Housing Programs. The Housing Case Manager is responsible for the assessment and collaborative development of the participant’s individualized plan of care using the Strength Based Model. In addition, the Housing Case Manager is responsible for the coordination of services and assistance needed to meet identified goals, with the overall objective of securing and retaining housing, and access and engagement to recovery services.
ESSENTIAL POSITION RESPONSIBILITIES :
Meet deadlines and ensures accuracy of various reports/paperwork, mileage sheets, and electronic timesheets.
Maintain accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.
Assist participants in setting individualized recovery goals by utilizing the strengths assessment process and update quarterly.
Complete treatment plan using goals identified from strength assessment and update quarterly.
Assist participants with achieving housing/education/employment/recovery goals by employing strategies derived from goal worksheets.
Ensure participants are able to meet ADL’s by assisting, coaching and skill building around daily living, social skills and medication management skills.
Meet with participants to coach and provide instruction necessary to meet identified goals and work toward recovery, reintegration, relapse prevention and community tenure.
Monitor status of participant and progress of ongoing services to evaluate effectiveness of services, provide level of personal and other supports consistent with consumer status, and provide revision as necessary. Notify team of changes in client status and document in progress notes.
Act as a liaison, client advocate, between the participant and other referral services required to meet identified goals based on the needs of the participant.
Support and assist participant with housing option searches, housing applications, linkage to community resources to securing and sustaining affordable permanent housing.
Assist participants with entitlement advocacy and application processes including for SSI/SSDI, food assistance programs, medical cards and others.
Assist participants with crisis situations, anger management, landlord/tenant dispute resolution and development of crisis WRAP plans to address symptom management and skill building for conflict resolution.
Assist participant with budget development, budget management and education on the benefits of a budget.
Attend and participate in individual, group and field supervision, as established by the program.
OTHER POSITION REQUIREMENTS :
Maintain acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed.
Exhibit appropriate level of technical knowledge for the position.
Produce quantity of work necessary to meet job requirements.
Work well with a team, keeps others informed of information needed and treats others with respect and maintain a spirit of cooperation.
Maintain effective and professional verbal and written interactions with peers, customers, supervisors and other staff which also includes effective listening skills. Uses diplomacy and tact in dealing with difficult situations or people.
Demonstrate the ability and willingness to handle new assignments, changes in procedures and business requirements. Identify what needs to be done and takes appropriate action.
Complete assigned work, meets deadlines without reminders / follow-up from supervisor or others.
Perform work conscientiously with a high degree of accuracy.
Collaboration with and data management in HUD’s coordinated homeless management information system (HMIS).
Collaboration with partnering agencies and CIT Teams for mobile response and stabilization services.
Meet goals and objectives as mutually agreed upon during last performance review (if applicable).
POSITION REQUIREMENTS : The Housing Case Manager is expected to have a Bachelors degree in a human service field or four years of equivalent education and/or experience working in the human services field. Computer literacy required. Strong work ethic required. Ability to take direction. Preferred areas include knowledge of community resources, housing alternatives and vocational services; experience working in the community with individuals who have serious and persistent mental illness and substance use barriers; ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with clients, constituents and staff. A valid Kansas driver’s license and access to personal vehicle required.
