Summary of Position: This position is responsible for networking with community businesses and resources to plan and implement center and community based activities. All duties listed below are essential functions of the position. General Responsibilities 1.Participate in the day to day operation of the Life Enrichment (LE) Services programs 2.Maintain and evaluate materials, supplies and equipment for program purposes. 3.Assist persons served during lunch and break times to assure their meals/snacks are prepared properly and following their diet requirements. 4.Assist persons served with eating, as needed. 5.Assist with cleaning program areas at the end of the day. 6.Communicate any program concerns to the LE Program Supervisor. 7.Report and properly document all accidents, illnesses and observations on an Incident Report form and turn in promptly. 8.Develop positive supportive relationships with the persons served. 9.Communicate using person first language. 10.Use and encourage communication with persons served that is positive and respectful, and promotes independence. 11.Display the energy of caring by being active in meeting the needs of persons served. 12.Promote the independence of persons served. 13.Network with community businesses and community resources to coordinate recreational, educational, and volunteer activities for persons served. 14.Coordinate transportation for community activities. 15.Develop monthly community and center based activity calendars to submit to the transportation department and program supervisor. 16.Coordinate with program supervisor to ensure adequate funds are available for activities. 1.Provide training, support, direct care, and safety of the persons served 2.Actively engage person served in meaningful activities and training opportunities. 3.Individualize services and supports to provide the maximum benefit possible to each person served. 4.Plan activities to meet goals and objectives as stated on the Person Centered Support Plan (PCSP). 5.Implement Behavior Management Plan as specified in the PCSP, record data and turn in as required. 6.Implement small group activities that are meaningful and educational in nature while center based. 7.Participate in community based activities that include both training and recreational activities with input from persons served, as needed. 8.Network with other community resources to support the persons in recreation activities they have selected. 9.Instruct and assist persons served with their self-care, communication, personal and social skills. 10.Train and assist persons served on safety procedures. 1.Transport persons served to meet programmatic needs 2.Arrange and/or provide transportation for community outings. 3.Assist persons served getting on and off the vehicle, as needed. 1.Communication responsibilities 2.Develop positive supportive relationships with the persons served. 3.Communicate using person first language. 4.Use and encourage communication with persons served that is positive and respectful, and promotes independence. 4.Display the energy of caring by being active in meeting the needs of persons served. 5.Promote the independence of persons served. 1.Meet personnel and organizational requirements and expectations 2.Possess a valid driver\'s license. 3.Maintain all training certifications, as required.
