Job Summary and Qualifications
DUTIES INCLUDE BUT NOT LIMITED TO:
Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support
Research and advocate for appropriate public assistance resources for clients
Referral to community resources, assistance with socio-economic problems
Communicate with client’s care team
Provide crisis intervention (referral to DCF, provider, etc as needed)
Coordinate treatment planning with specialists and outpatient providers as needed
Evaluate current patients for appropriate social work services
Judge relative costs/benefits of potential social service and health care actions and choose the most appropriate one
What qualifications you will need:
Experience working with clients that are affected by issues including neglect, abuse, domestic violence, substance abuse
Minimum of 2 years prior experience
Bachelor's Degree (required)
Clinical Mental Health Counselor, or Clinical Social Worker (MSW), or Licensed Clinical Social Worker (LCSW)
State licensure or certification commensurate with degree
